For what purpose do we process your data, on what legal basis and for how long?
In order to perform the contract pursuant to Art. 6(1)(b)/(f) GDPR for platform data; Art. 9(2)(h) for health data processed by your clinician. Your data will be processed until the completion of all factual and legal activities necessary to perform the contract, and then until the claims arising from it expire.
In order to perform obligations under the law (e.g. tax settlements), pursuant to art. 6 sec. 1 lit. c) GDPR - for the period to fulfill these obligations and for the period required by law.
In order to implement our legitimate interest (Article 6(1)(f) of the GDPR), i.e.:
for the implementation of current administrative and business tasks, to ensure the protection of information, the disclosure of which may expose us to damage, to establish, pursue or defend claims related to business activity. Your data will be stored until the legitimate interests constituting the basis for this processing are fulfilled or until you object to such processing.
Who we share your data with
Only the specialist whom team you joined and entities that cooperate with us, e.g. entities providing IT services, legal services, accounting services.
You can request access to personal data, rectification, deletion or limitation of processing, and you have the right to transfer data. You also have the right to lodge a complaint with the supervisory body (President of the Personal Data Protection Office, Personal Data Protection Office, ul. Stawki 2, 00-193 Warsaw). Providing personal data is voluntary, but necessary to conclude and perform the contract. If you do not provide us with your data, it will not be possible to conclude a contract.
